LDL education: (1) never confuse the term LDL, the name of a specific lipoprotein with LDL-P a particle concentration metric. (2) LDL is named by its density via ultracentrifugation (3) What is LDL size, peak particle size or Pattern? (4) How many LDL particles exist per liter of plasma?

What is the size of the thrombus in a coronary artery that can send one to Heaven or the CCU? Coronary artery lumens are quite small, and a cholesterol-rich plaque rupture or erosion will induce a thrombus. A tiny thrombus will partially block (cause ischemia) or totally block (cause infarction).

To further confuse the EPA vs EPA+DHA story. Here is new data from STRENGTH showing EPA + DHA reduced MACE in Asian patients. Authors had no reasons but perhaps due to Asian incidence of IR and TG issues. www.atherosclerosis-journal.com/article/S002...

DHA is crucial for maintaining brain neuron integrity and may inhibit tau phosphorylation. High doses (0.9–1.8 g/d) of DHA/EPA have shown improvements in cognitive function, while low doses (0.3–0.7 g/d) did not. www.sciencedirect.com/science/article/pii/S0002916523463204?via%3Dihub

"Understanding the Lipid Panel" - next are insights on Non-HDL-C which refers (1) to the mass (weight) in mg of cholesterol within all of the apoB-containing lipoproteins per dL or (2) the number (in mmols) of cholesterol molecules in all the apoB-particles per L.

Insights on HDL-C reported as (1) mass (weight in mg) of all of the cholesterol molecules within all of the HDLs per dL of plasma or (2) the NUMBER of all of the cholesterol molecules in HDLs er liter [mmols). HDL-C or HDL-P have no relationship HDL functionality or reverse cholesterol transport

"Understanding the Lipid Panel" - LDL-C reported as (1) the mass (weight in mg) or the # (mmols) of all of the cholesterol molecules within all of the IDLs and LDLs circulating in a dL of plasma LDL-C is calculated, directly measured, or assayed as the gold standard assay termed beta-quantification

Revisiting my series "Understanding the Lipid Panel" - 1 metric at a time starting with Total Cholesterol. The lipid panel reports lipid mass concentrations but not the lipoprotein concentrations. In essence lipid concentrations are simply guesstimates or surrogates of lipoprotein concentrations.

Lipidologists: Nice study by my friends at NIH on the importance of HDLs, not LDLs, supplying cholesterol to steroidogenic tissues. HDLs in addition to Reverse Cholesterol Transport also do Forward Cholesterol Transport. Check with author @RZubiranS for reprints pubmed.ncbi.nlm.nih.gov/40767551/
