I mean, there is an additional level (higher doctorates) but it's not distinguishable by title (in some countries they use the title Dr Habil.)

It’s difficult to read about the 4th century and believe Christians then had no concept of religion and non-religion, they seem to have thought of little else

I would broadly agree: the concept of the secular (non-religion) was made possible by the rise of confessionality (i.e. Christianity), and the Reformation sharpened rather than invented the distinction
