Can there be fluctuations in cognition and mood early in PD? Do you ask about anxiety and dysautonomia? The answers may surprise you. Spoiler alert: Stop being hyperfocused on motor dysfunction. Spend more time w/ folks and you may be surprised what emerges –de profundis – out of the depths.

The closure of hospitals, especially in healthcare deserts is a blot on our healthcare system.

Crozer Chester Medical Center hosted residency training programs for IM, FM, EM, ObGyn and Psychiatry. These two hospitals saw nearly 80,000 ER visits last year.

Can cognitively unimpaired PD patients experience fluctuations in cognition and mood with dopamine replacement?

Yes: analysis reveals greater odds of fluctuations in anxiety (OR 7.5) and cognition (OR 2.8) with autonomic burden.

Unlike private practices, some academic dept. have $0 CME and at least one is prohibiting attendance

Academic leadership: Physicians stay for academic advancement despite poor autonomy, lower pay, lesser clinical support, more admin oversight. "Prestige" alone won't suffice for retention.

Most people get care where (and if) they can. For those able & willing to travel, they follow the reputation of specific programs/ individuals.

With departments housing 100 or more neurologists, can such a ranking even be accurate? What is the purpose?
