Pharmacist here - the pharmacy doesn’t independently decide. If the physician wrote it for “quantity: 30 day supply x11 refills” they can’t give you more per fill unless the physician changes the script to “quantity: 90 day sup x 3 refills” - if that’s not the issue it’s definitely the insurance.
