In this issue, Vega Castro et al., in their EAACI Position Paper, provide an in-depth analysis of the skin challenge test’s key applications, risks, and outcomes in clinical practice, focusing on its role in monitoring efficacy of venom immunotherapy. Pfaar et al. review the real-life clinical experience with SQ grass sublingual immunotherapy tablets based on evidence from non-interventional studies. Woehlk et al. report data from a paediatric study to assess the real-world long-term effectiveness of all AIT products prescribed in a large public health system to children with allergic rhinitis, with or without pre-existing asthma. Arnau-Soler et al. identify molecular drivers of oral immunotherapy responsiveness through multi-omics profiling in immune cells. Okubo et al. demonstrate real-world effectiveness for sublingual allergen immunotherapy among school-aged children and adolescents. Pfaar et al. apply a patient-centric approach and provide minimal thresholds to be achieved after AIT compared to placebo to conclude a positive Phase III trial outcome. Bachert et al. investigate the safety and efficacy of a subcutaneous house dust mite (HDM) preparation in a randomized controlled trial using an optimal higher dose than the current maintenance dose. Roberts et al. evaluate the efficacy and safety of the SQ HDM sublingual immunotherapy tablet in children with HDM allergic asthma. Lahnsteiner et al. investigate whether differences in DNA methylation occur in birch pollen allergic patients undergoing six months of birch pollen AIT, assess alterations in methylation-based blood cell type composition, and correlate DNA methylation to serological AIT biomarkers. Arzt-Gradwohl et al. evaluate the safety and efficiency of a 7-week immunotherapy protocol with aluminum hydroxide adsorbed bee venom. Alessandro et al. present preliminary results of their study investigating the safety and tolerability of a 3-hour build-up phase with Hymenoptera venom depot extracts.

Food proteins from peanut, egg, and cow's milk were detected in human milk with variable kinetics. Ara h 2 in human milk was capable of IgE-crosslinking as demonstrated with the basophil activation test. Higher levels of peanut protein, Ara h 2, were found in atopic mothers compared to non-atopic mothers.

Novel paediatric data showing that the safety profile of SQ HDM SLIT-tablet is similar to that established in adults. This issue opens with an editorial by Li et al., highlighting the inaugural EAACI Hong Kong Allergy School where global experts in drug allergy gathered under the theme “East Meets West” to call for harmonized international definitions of drug hypersensitivity and standardized criteria for identifying low-risk penicillin allergy, underscoring the urgent need to reduce mislabelling and advance safe, evidence-based delabelling practices worldwide. Leonardi et al. provide a comprehensive overview of the clinical presentations, diagnostic approaches, and treatment strategies for drug-induced ocular adverse events, integrating the latest literature and clinical guidelines. Cortellini et al. present data and recommendations on the characteristics of hypersensitivity reactions to antiplatelet drugs and the associated diagnostic procedures, to maximize their safety and effectiveness. Gelincik et al. review current knowledge on the pathomechanisms of hypersensitivity reactions to quinolones, including diagnostic tests and cross-reactivities, and propose a practical diagnostic approach for clinicians. Hutten et al. highlight significant discrepancies between studies on cross-allergy to beta-lactam antibiotics, particularly concerning recommendations for safe alternatives. Labella et al. investigate the utility of direct single-dose drug provocation testing without prior skin testing in adult European patients at low risk of penicillin allergy. Bilgic-Eltan et al. analyze the discrepancies between suspected and confirmed drug hypersensitivity in a well-defined cohort of patients with inborn errors of immunity.

This study investigates the longitudinal effects of #dupilumab on the nasal passage and gastrointestinal microbiota in patients with diffuse type 2 chronic rhinosinusitis with nasal polyps (#CRSwNP). CRSwNP patients show altered nasal microbiota composition compared to healthy individuals. Dupilumab treatment is associated with a shift of nasal microbiota in CRSwNP, with increasing beneficial genera (Lawsonella, Corynebacterium, and Dolosigranulum). Gastrointestinal microbiota remains unchanged during dupilumab treatment.
