📍 Ankylosing spondylitis with fracture.

🩻 X-rays show dagger spine, squared vertebral bodies, bamboo spine, and fused SI joints.

🗡️ Broken dagger on AP marks an L3 chalkstick fracture, confirmed on lateral and CT — a classic brittle spine injury.

Achalasia on a barium swallow: very dilated oesophagus (UK spelling as I practice in the UK!), barium mixing with food residue giving a bubbly appearance, smooth narrowing to a very tight lumen at the distal oesophagus, known as the 'bird beak' sign, with little barium passing into the stomach.

🎯 History: Abdominal pain.

🎯 CT: Bilateral rectus sheath haematomas with increased muscle density. No definite bleeding point (no arterial phase). No intra-abdominal extension.

🎯 Usually unilateral, most often spontaneous in patients on anticoagulation.

📌 Acute appendicitis on CT:
🟣 Gas + calcium appendicoliths in appendix
🟢 Caecal pole = key landmark
🔴 Dilated, thick-walled, fluid-filled appendix
🟠 Surrounding fat stranding = inflammation
❗️Look for perforation signs: abscess or free gas
