Ah the form has culminated in a short text message reply which doesn't fully address my needs, a push to request private support and me in tears. Brilliant. Excellent work all round.
It took me 15 minutes to complete the form, by the time I’d done it and gone back to the beginning the form was closed again
You have 2 options: write your symptoms into a form (at 8am) or try and get an appointment at (8.30am)
I do not understand what possible reason you would have to close a form like this...

The worst anxiety dream, I can’t remember how to get to my office, then when I eventually find it every computer I log on to won’t let me access my calendar, I know I have a meeting but I can’t get to it, people keep distracting me or having breakdowns around me
