Every org has three sets of intentions:
➡️what it openly declares
➡️what it quietly practices
➡️what it actively discourages
Surgical safety improves best when these are ALIGNED, not when policies mask conflicting incentives and expectations.

Seems telling that the ShameLab started out in #OBGYN context. We desperately need a culture change in how we handle emotional reactions to adverse events / complications.

These doctors want to break the cycle of shame and blame in medicine
Shaming is common in medical school and unfortunately, sometimes in the doctor's office. Some doctors are trying to teach medical students awareness of the problem so they don't perpetuate the cycle.

One camper got so homesick she asked to be pickd up early.
I was devastated. I cried so hard I sat on the steps behind the dining hall to catch my breath.

That’s one in 100 opportunities for error.
To get to 10^⁻3 or better, we need more than skill.
