We welcome this new definition of Systems Generated Trauma (SGT). We see too many incidences of harm done to families by persistent blame, inaction and/or cost-saving over adequate care. Real lasting harm is being done to some of the most vulnerable members of our society

sounds like such a trope but after all I've seen working with "severe & persistent mental illness," truly the most violent & tragic stories, it makes me want to grab some people's faces in my hands & urgently whisper to them:
