Very hot take! Our CLABSI rates were so high and we had such a high prevalence of CVCs being used due to massive AVF delays. We've worked really hard on it and got CVC rates and now finally starting to see lower BSI rates. How quickly do yours get permanent access after starting?
But isn't that exactly what a suboptimal start is? (we have loads of those too...)

Recommendations:
-Limit nonessential meetings to core hours (eg. 9-4) recognizing that time outside this window is not equitably accessible
-Offer asynchronous or hybrid options for professional development & decision-making forums, reducing reliance on physical presence as a proxy for engagement

30M with aggressive crescentic IgAN. M1E0S1T1C2. 4g proteinuria with Cr 160 umol/L but rising. Induction with IV CYC and pred. Now 1g protein but Cr, now 250 umol/L. Not sure what next :( maybe repeat biopsy? @nephrowhitt.bsky.social @hswapnil.medsky.social @askrenal.bsky.social

My patients live at the margins of society. They don’t have much. Just getting to dialysis three times a week is a big ask. Taking from them to fund tax breaks for the rich is just cruel.

Sorry late to the post but have seen dramatic hypophosphataemia post FCM in a tx patient. Now much more cautious re: timing in these patients!
