Yesterday I had the opportunity to present at #HEPS2025 in Dublin, sharing my PhD findings on how older people enact medication self-management behaviours during the hospital-to-home transition. It was great to be part of such a thoughtful space. Thank you to everyone who engaged with the session!
📌 To support future practice and research, we propose key recommendations for the development of more practical, person-centred assessment tools.
🛠️ While several useful tools were identified, there is a notable gap in implementation in clinical practice, often due to the time and complexity involved in administering them.
🏡 This review identifies existing measures and tools used to assess older people’s capability to self-manage medication as they move from hospital-to-home.
