Anathem (who is trans) is defining his transition goals in the Drake meme format. The first row shows him rejecting depictions of traditional human masculinity (such as facial hair, big muscles, having male genitalia). The second row is him approving of an image of him with multiple monstrous limbs coming out of his open back. He desires to be an eldritch horror basically.
