You're looking at it from a young person's perspective. When people get old (like me) doctors want to see us more frequently. He clearly has medical issues and should be seen more than once per year. Unfortunately, we won't be told if his problems are normal aging or more serious.

The scenario isn't new. Before ACA with Medicaid expansion, poor Americans and undocumented immigrants used hospitals for their primary care because hospitals can't deny care due to inability to pay. Hospitals would write off amounts they couldn't collect and charge more for people who could pay.
