This is a real person with a real condition in front of you, and it’s not their fault or yours.

Here’s what to do:

* Make the diagnosis on the basis of positive clinical features
* Call it FND
* Explain why you made the dx
* Stay agnostic as to cause, at least on first appt
* Find a treatment path in your area if possible
* Connect the patient w resources

This is frustrating to me because these are solvable problems.

They’re already solved for other disorders because we don’t treat other disorders like this weird thing that defies the laws of medicine.

We should do the same with FND!

7. Finally, there’s the Ping Pong experience, where people are repeatedly sent back and forth between specialists who each say, “this isn’t my job for X reasons.”

6. People with FND are often left without treatment.

The condition likely won’t get better on its own. It should be treated, just as any other brain disorder naturally should be treated so people can live better lives.

5. Conversely, patients are told their symptoms are “non-organic” or don’t have a cause, or don’t involve biological changes.

How could that literally be possible? All illness, whether mental, physical, whatever, involves changes to physiological processes of life. Otherwise it couldn’t exist

4. Unsupported causal attributions.

Patients are told their symptoms must be the result of stress (or trauma, abuse, or in some sexist cases, that a woman needs to get married!!).

FND has many causes. Lack of husband isn’t one. And presence of FND symptom doesn’t tell you why it happened.

3. Patients are often not being given a clear diagnostic label.

It’s “Functional Neurological Disorder.” 👍

Subtypes include Functional/Dissociative Seizures, Functional Movement Disorder, etc.

Please refrain from spontaneous dx linguistic improv. It’s weird and confusing to patients

2. Patients are often (still) being given the FND diagnosis when “no disease is found.”

This is not how it should be done.

FND needs to be diagnosed based on positive signs / clinical features that establish that FND is present.

1. People aren’t being diagnosed with FND when they clearly have it, due to lack of training for medical professionals.

Doctors don’t recognize it, so they don’t diagnose it. It often takes years - sometimes a decade or more - to get a dx that could be accurately made on the first visit.

There are a lot of difficult challenges around FND (understanding brain dynamics, developing new treatments, etc).

But the most *important* problems are much more easily solved - and yet I hear from folks about these same issues in the community over and over.
