I'm finally FINALLY close to getting my CPAP/APAP machine to start treating my sleep apnea 🙌

Now I need to start the process of finding a mask that works well for me and honestly it feels a little daunting 🥴

Careful with the APAP -- don't go over 3g in 24h (so no more than 9 pills in any 24h period). If it's not managing the pain enough, call your doctor and ask for oxycodone without the acetaminophen in it (there's one hydrocodone-only formulation but it's not widely available)

I can not tell you how many times I have tried to explain to patients that Tylenol, APAP and acetaminophen are the same. Tylenol is just a BRAND. Drives me nutty sometimes because they know everything.

As a post script, I also want to say that I find the “medically necessary” language around APAP use ridiculous and insulting. Do they think pregnant women are taking acetaminophen for fun? Or mistaking the pills for candy?

incidentally, APAP has several extremely unusual effects for any medication on account of it acts similar to a SSRI or SNRI but for endocannabinoids. people dosed with tylenol experience reduced symptoms of religious awe. anecdotally i tend to take it as a rescue for religious psychosis

This is a really great example of how to terrify people with the selective presentation of information and omit critical context. 🧵

First: The dose makes the poison. Acetaminophen (APAP) can indeed be a cause of liver failure at higher doses. However, about half of these overdoses are intentional.

2/ Given how common APAP use is, teasing out the effect of APAP vs other known risk factors is very challenging.

2nd, there is no biologically plausible mechanism by which occasional APAP use causes ASD/ADHD. APAP is certainly very toxic at high levels or with chronic use, but a couple of pills?

Okay, so NSAIDs seem to be worse than APAP. So what other pharmacological tools do we have to treat pain in pregnancy? That’s basically it as far as over the counter stuff. There are a handful of other options but generally they are only effective for specific types of pain e.g., muscle relaxants…

If you know me you know the beef I have with acetaminophen.

I *still* hope that Kenvue unleashes their VERY well-funded fury onto RFK and the administration. (Esp because RFK keeps saying “Tylenol”.)

(There is no convincing connection between APAP and autism, btw.)

Acetaminophen (aka APAP aka paracetamol) is rough on the liver, so you need to be careful with it. My gripe is that it gets added to SO MANY combo drugs (DayQuil, OTC migraine cocktails, etc) that it becomes super easy to overdose unwittingly.

It is, however, my choice for breaking fevers.
