Unfortunately companies, especially regulated ones, automatically include all sorts of stuff that the sender never even sees.

It was signed for at their mail processing centre, then vanished into thin air. And you can't chase for 16 weeks (or more), so it took 6 months just to establish that it needed to be resubmitted.
