Is this primarily common in English speaking countries? In Switzerland, patients would never address their physician with first names - always Dr. Last Name. When mistaken for a different role - e.g. with racial or gender bias - then Mr./Mrs. Last Name, but never a first name.

No significant difference was found in overall PPCs between ‘low etCO₂ (below 35 mmHg)’ and ‘normal to high etCO₂ (≥ 35 mmHg)’ patients (20 % vs. 19 %).
