Because you look and see a skull head, but it's not a skull head, it's a skull hat with some nobody face under, or in it.
Also this is specific. The cheek bones from the mask-head-thing go near his mouth, and it looks like he's doing the side-cheek smile thing that Spongebob does that people hate.
