sorry/not sorry. i am just surprised at how consistently the "14 days of abx for pneumonia" is recommended.

it makes me wonder if there is a secret respirology literature on the topic that supports this duration.

we encountered a similarly dramatic case of disseminated sporotrichosis during my ID training.

he was thought to have sarcoid and received glucocorticoids. only later did a dx of hairy cell leukemia become apparent.

i saw an unforgettable case as a pgy1. we actually aspirated a lesion in ED and found GNB on gram stain.

there are times when the deep dive is warranted, but including every detail in a case presentation (where most of the details are already in the note) is often superfluous.

and yes, there is skill in being able to distill a case into its core features.
