For the sake of discussion, EMPA-KIDNEY (or any SGLT2i trial) excluded pts with serious immunosuppression... And since we look at subgroups, there is no difference in genital infections when accounting for primary kidney disease #nephjc

1/ As a Romanian, and Eastern European this is funny to me- meaning by any ranking, my country has highest alcohol consumption worldwide. I grew up assisting my great grandparents preparing moonshine (main source of methanol), and learned how intoxication looks

Basically, KDIGO has the same level of recommendation in both cases for RASi in patients with CKD and moderate albuminuria or measuring proteinuria and Cr for CKD diagnosis as for low-ish protein consumption in CKD. #nephjc

There are case series reporting. We had a pt, from rural area, and didn't took her IS for a while (she didn't remember), didn't come to cotrols for about 2 years. We only found out he was not on IS, because her T0=0. Cr after 15 yrs of KTx= 1, 1 mg/dl #NephJC

I think the general message is beyond ACHIEVE:
1. Big, rigorous trials are possible in dialysis.
2. It is more about doing than complaining about the lack of quality studies.
3. Negative studies serve us better lessons than pozitive ones #NephJC
