And it kills me when patient says something like “oh the hospital doc said that medicine was hurting my kidneys”
And then there’s reluctance to resume.

So I try to teach trainees to be mindful of their choice of words if holding RASi is indeed necessary.

Stopping RASi should be the last resort.

Fruits & vegetables 🍊🥑🥦are ⭐️NOT⭐️ the problem.

❌Myth: restricting dietary K is useful to prevent hyperkalemia

❌Myth: Avoid fruits/veg - the main source of dietary K+

Dietary potassium can be good for you.
AND there is little to no correlation btwn dietary K intake & serum K level (even in pts w/ CKD).

HyperK-related RASi discontinuation or down-titration was a/w higher risk of cardiorenal events vs maintained or up-titrated RASi.

And when RASi gets held, we’re not good at restarting them.

Our concern is safety, hyperK can be dangerous. But is stopping RAS really safe… not really.

I’m asking this to trainees in the CKD clinic every single time.
Wait, why was this patient’s ACEi/ARB stopped?!? 🧐

What happens when we stop ACEi/ARBs?? Nothing good…

RAS blockade is protective

Decrease CV events
Decrease CKD progression
Decrease HF hospitalization
Etc etc
#Kidneywk

And the effect is maintained even with low eGFR!
