The ACA mandates that all health insurance plans must cover all vaccines recommended by the CDC Advisory Committee on Immunization Practices (ACIP).
So what happens when they're no longer recommended is they're no longer required to be covered.
Other rules elsewhere are also tied to ACIP.

Had a horrible time getting their treatment covered because their insurance was following the rules from one of those states that have effectively outlawed transition.
