Mostly use bisoprolol in the evening in HD

interesting paper on this eternal bugbear: what antihypertensives to choose in HD patients. my practice is also to move beta blocker in the evenings when patients are already on them, especially when patients had intradialytic hypotension. do other folks do this?

I would love a smart sock for heart failure and nephrotic patient - a device that monitors peripheral oedema continuously and remotely and helps us guide treatment from the distance. I have seen papers on this but dont think this is currently available commercially

as a young nephrologist I was forever fascinated by vasculitis and pulmonary-renal syndrome. Here is an interesting report on this topic. take home message: pulmonary renal syndrome isn't always vasculitis

Went to a Nina Simone programme on Saturday. It was super but also made me want to listen to the original again - incredible. Here the famous concert in Antibes in 1965 - incredible 🙏

Amazing stuff. Bought langoustines, a Turbot for the freezer and two skate wings 👨‍🍳

Bought some fish off the boat today 🙏🐟👨‍🍳

I must have just under 100 on finerenone and perhaps 5 where I had to stop due to hyperkalemia

For comparison: my working cocker spaniel

A highlight in todays video clinic: an in-depth discussion with a patient about her Kooikerhondje. To the uninitiated: an unusual Dutch spaniel breed 🇳🇱. Another benefit of SGLT2 inhibitors and finerenone. More stability. More time to talk about dogs 🐾

This was amazing. Maybe next time I will try and source Toulouse sausage 🇫🇷
Wine: Zweigelt 🇦🇹
Struggled a little to keep up with the onslaught of flavours. Maybe a Minervois or Corbieres would have been better. Or chateauneuf du pape? 🍷

The kitchen smells like an auberge in Toulouse or Carcassonne in an evening. The spaniel has woken up and started salivating. The fridge looks empty. We are out of garlic. All good prognostic signs for Cassoulet 👨‍🍳🇫🇷

I have had some positive news this week so decided to make a bit of an effort with cooking. Today: cassoulet. I toyed with making an atorvastatin veloute with it but decided against it. Vive la France 🇫🇷

Question to my followers who like classical music: does anybody know a version of this that is perhaps a little more restrained with emotion?🎶🎹

This quote is I think also very timely:

“Totalitarianism in power invariably replaces all first-rate talents, regardless of their sympathies, with crackpots and fools whose lack of intelligence and creativity is still the best guarantee of their loyalty.”

I think its now pretty much a given that the next government will be Reform. The more interesting question I think is this: another five years down the line standard of living is still no better and more brutal cuts have happened - what happens then 🤔🙁

I usually make this with smoked belly pork from Romania 🇷🇴 but my other half bought something else instead 🙁

We have declared autumn in our house. I made the first pea soup of the season this week 🧑‍🍳

Wine: ten year old Bordeaux. Nice, but in hindsight I would have liked something slightly more herby like a Minervois or a Corbieres 🍷

Amazing stuff - I think the extra mushrooms gave it more depth 👨‍🍳

Smell in the kitchen is amazing. I had some dried porcini and morels which I used. Very mushroomy Boeuf Bourgoignon 🍄‍🟫
