Slightly Reliable Engineer based in Amsterdam,NL.
Learning socio-technical something.
Explaining robust-yet-fragile in a community of HSE practitioners and contrasting it with Hollnagel's RAG. Boy! This is way outside my competence envelope.

Just learned that the operational resilience minimum standards at my current workplace is built on Eric Hollnagel's #Resilience Assessment Grid. I just got jet engines under my wings!

That's the law of fluency. I didn't make it up.

Beyond just the language setting ourselves up for failure, "root cause" have deeper issues. For me it triggers anxiety everytime i hear it. @allspaw.bsky.social has articulated it very well the issues here. Recommended material for SRE-201: github.com/readme/guide...
What we talk about when we talk about ‘root cause’
Instead of finding the ‘root cause’ to incidents and issues, @allspaw says it’s more accurate to try and break down what created the ‘perfect storm’:

@mccricardo.bsky.social just spoke about incident response at #FOSDEM. It's a new dawn of SRE and related crafts at #FOSDEM beyond tools and framework. Folks in the resilience engineering community, let's go with devroom proposal for FOSDEM 2026.

Folks visiting #FOSDEM, specifically #SRE and adjacent roles, want to talk about the state of incident postmortems, human factors, what you learned from incidents etc? Let's do an unofficial #BoF #LFI

As an Engineering Manager/peer engineer do you put on a poker face while #interviewing a potential candidate? I've been just me in all of my interviews but today I went to the other side of the table and faces stone cold poker faces. 🫠

Next Monday, 24-Nov-24 is the third anniversary of the infamous #log4shell vulnerability disclosure. Remember the scramble to identify what in your estate has the dependency & are exploitable? How did your discovery & governance game improved since then?
