Incidence, Prevalence, and Mortality of Interstitial Lung Diseases in Alberta, Canada: A Population-based Study

www.atsjournals.org/doi/suppl/10...

🧵👇

📣 TL:DR Incidence stable, prevalence rising, improved survival for ILD & IPF over 10 years.
Numbers much higher than prior reports for ILD & IPF.

As a past associate and deputy editor - the best thing is to put a sentence that you did not review the statistics and that your review is contingent on a rigorous statistical review by a stats reviewer
Yeah, good question.
I guess in the end it’s up to the journal and their level of integrity to decide.
You can only address what you know (and if you don’t understand the stats, neither will 95% of other reviewers!)
And it’s tricky because you can’t see the whole study until you accept the review.

Yes to statisticians in peer-review & project design and analysis, I wish scientific institutions would understand how crucial they are
I feel that the problem of highly complicated statistical methods is only getting bigger with studies harnessing biobank data, meta-registries, large databases etc.
Also according to him, “kids” should have babies against their will but not be held accountable for this
If I don’t understand methods I say so & suggest statistical review.
Probably every manuscript should have statistical review. I think they do that at Thorax as a policy but not sure about other 🫁 journals?

(Similarly every manuscript should have a biostatistician but we don’t all have access!)

I receive more and more #PeerReview requests where the subject is in my area but the statistical methods are much more complicated than I am comfortable assessing

#MedSky #PulmSky folks, what do you do in this case: accept or decline?
How do you make sure you don't miss something wrong?

Fantastic undertaking by @kerrijohannson.bsky.social and team to talk about an important aspect of people's lives which doesn't get the deserved attention
Well done!! 👏👏👏👏
As always you are a trailblazer, Kerri my friend! 💪

We are eternally grateful to all the patients who contributed to this study, entrusting our team with sensitive topics

Thank you to CARE-PF registry, PROFILE cohort, and all international collaborators who supported this work 😊

Sex matters to patients with pulmonary fibrosis

They are impacted by their dyspnea
PF impact on sex can cause distress
Sexual dysfunction is common
This needs to be addressed, and warrants further study

This should not be a taboo subject because it matters to people affected by PF
Figure 5
Seven patients participated in telephone interviews to discuss the impact of PF on sex and sexual function

Themes that emerged:
🫶 Relationships
😊 Quality of Life
🫁 Impact of PF on sex
🫤 Oxygen use

Several important representative quotes and thematic examples in the full manuscript
Figure 4 of the paper summarizing key themes
With help from colleagues, we sent a multilingual international online survey (English, French, Dutch, Spanish) to assess sexual dysfunction using the validated CSFQ 📝

140 patients graciously completed this online survey 🙏

Sexual dysfunction was COMMON
Affecting 83% of females and 86% of males
Table 3 from manuscript showing key survey results
Of 225 IPF patients in PROFILE UK cohort who completed the SPARC questionnaire, 23% reported distress due to the impact of PF on their sex life

More commonly distressed by this were men, younger age, lower DLCO%, worse cough
Patients with PF are dyspneic with sexual activity

Of 2759 patients from CARE-PF registry completing the UCSD-SOBQ, 74% reported dyspnea on Q21

Dyspnea more common in men, those on O2, with lower FVC% and DLCO%, and higher cough visual analog scores
Histogram showing 74% of patients had scores >0 meaning dyspneic
In this study, we sought to characterize the impact of PF on sex and sexual function across broad patient populations

4-pronged approach:
🫁 Impact of PF on dyspnea during sex/sexual activity
🙁 Distress caused by PF impact on sex life
🌟 Sexual dysfunction (physical)
🎙️ Qualitative interviews
Pictorial representation of study approach
For many adults, sex & sexual activity are important

Patients with PF are often limited by breathlessness, cough, fatigue, deconditioning, O2 use

Prior studies suggest that PF may impact sex & sexual function, but limited patient populations, generalizability and focused mostly on physical issues

The 400 richest Americans are now worth a record $6.6 trillion.

The entire bottom 50% of America is worth just $4.2 trillion.

Read that back.

When 400 people control more wealth than half a country’s population, we have a very serious problem.
